Registration Form

Information for Parents

 

Registration is on a first-come, first-served basis. Classes usually fill up quickly, so please register early. We will maintain a wait-list in the event any students decide to drop from a course in which they are enrolled. 

To register, print and complete the registration form below, attach your payment, and mail or deliver it in person to the address indicated.

 If you have any trouble accessing the links above, or would prefer to have a paper copy of the registration form mailed to you, please email slsturge@indiana.edu to request assistance.

 All registration will be confirmed via email or US mail prior to the start of the program. If you have not yet been contacted and want to confirm receipt of your registration materials, please contact the Science Education Secretary, Sara Sturgeon at slsturge@indiana.edu at 856-8129.
